C-TAC is working across four domains – care, cost, caregiving, and community – to develop interdisciplinary responses to these complex and increasingly pressing problems. Whether through our work creating an advanced payment model, reaching out to new and underserved communities, working with Members of Congress on legislation, or educating clinincians on how to have end-of-life conversations, C-TAC is on the forefront of creating solutions to bridge the gap between the care people living with advanced illness want and the care they actually receive.

Attending the summit will mean engaging with the people working to solve the core problems in advanced care. This year’s agenda will include a variety of smaller breakout sessions which will delve more deeply into specific participant-requested arenas, including how technology is changing medicine, how to provide support to family caregivers, measuring success in the movement, and more.
