About the Session

This panel will focus on how engagement and collaboration can help support and grow a learning health system for implementation of innovative models for advance care planning and palliative care using examples from research, health care delivery, and payment models.  We will reference initiatives such as the Patient-Centered Outcomes Research Institute’s Funding Initiative for ACP and palliative care which has also fostered a learning network, UCLA’s Advance Care Planning and Services initiative, and the Blue Cross/Blue Shield Serious Illness Work Group.
