About the Session

With increasing pressure to deliver high-value care, innovative organizations are transforming their systems of care to ensure that care is consistent with patient goals and values.  Advocate Aurora Health has engaged with Respecting Choices to ingrain deep systems change with use of data and evidence to ensure that they ask, understand, and honor their patients’ wishes.  To strengthen the value proposition for this work, Advocate Aurora Health has engaged Quantified Ventures to support program financing, accountability to outcomes, and scale. This three-way partnership represents a natural progression toward meaningful and sustainable systems transformation.
