Community Collaboration to Reduce Fracture Risk Caused by Osteoporosis

Only 20% of patients who experience a fracture caused by osteoporosis are assessed and or treated for the underlying cause for that fracture. Due to the fact that there is a 5x higher risk of another fracture within 12 months following a fracture, it is imperative that we find a way to close this care gap to improve the health outcomes for this patient population.

As our Summit comes to a close, C-TAC Executive Director Jon Broyles will offer final thoughts, moments of inspiration, opportunities to communicate and share. This will be capped with a preview of the 2021 C-TAC Summit on Advanced Care Illness, scheduled to be held in Washington, DC in October 2021. We will conclude with a blessing from Rabbi Richard Address.

We will be kicking off the final day of our Summit by identifying lessons learned during the previous day’s highly-interactive visioning labs. We’ll also hear calls to action from a leading voice in our field as well as a patient advocate. We’ll also discuss strategies for measuring success and take a closer look at C-TAC’s initiative in Arizona, putting our ACT Index to work for people and providers on the ground.
