TRANSFORMING CARE FOR PEOPLE FACING SERIOUS ILLNESS

We are gathering national changemakers working to improve quality of life for people living with serious illness, and their caregivers.

It takes leadership and action from frontline clinicians, community leaders, administrators, payers, and policymakers to positively transform the health care system. We are all aligned to improve health equity and quality care for all patients and their caregivers through policy advocacy, care innovation, patient empowerment, and clinical skill building.

C-TAC and the Center to Advance Palliative Care (CAPC) are joining forces to host a uniquely immersive, multi-track experience of talks, panels, interviews, and roundtables to ensure a mix of learning, sharing, and networking. Leaders from all sectors focused on advancing person-centered care for people living with a serious illness should attend. This includes leaders of health systems, payers, and community-based organizations; clinicians; family caregivers; policymakers and government experts; consumer advocates; faith and spiritual leaders; and philanthropic supporters.
